About OHTs and Important Links
The Ontario government is building a connected health care system centered around patients, families and caregivers. These changes will strengthen local services, making it easier for patients to navigate the system and transition between providers.
Ontario Health Teams are a new way of organizing and delivering care that is more connected to patients in their local communities. There are 58 Ontario Health Teams across the province providing full provincial coverage and ensuring that every person in Ontario can have the support of an Ontario Health Team. The goal is to ensure that everyone in Ontario can benefit from better coordinated, more integrated care. Under Ontario Health Teams, health care providers (including hospitals, doctors, and home and community care providers) work as one coordinated team – no matter where they provide care.

Become a Partner of the Durham OHT
The Durham Ontario Health Team (OHT) is a collaborative group of organizations and patient, family and care partner advisors working in partnership to improve access and delivery of coordinated health services. The group is comprised of partners from across the care continuum, including primary care providers, hospitals, mental health and addiction services, social support services, home and community care support services and long-term care.
The Ministry of Health (MOH) announced the province’s first OHTs in December 2020. The Durham OHT was among this group.
While the core Durham OHT partnership group is comprised of 18 signatory organizations, there is broad support from more than 70 health care partners across Durham Region, representing various organizations in the health, social, education, and private sectors.
The Ministry of Health and Ontario Health through the Path Forward guidance, the Durham OHT is committed to ensuring that our partners represent the Durham region with continued focus to build our partnership and OHT.
Our Vision: The Durham OHT will become one connected system of health and social care and support for residents of Durham region; filling gaps in service, reducing duplication, improving patient, family and caregiver experience, and demonstrating a successful integrated model of health care provision.
Our Partners
The Partners will contribute resources (e.g., funds, people, capital, and facilities) to the shared priorities and accountabilities of the Durham OHT as may be agreed, such contributions to be made recognizing different abilities and depth in resources and funding. The Durham OHT recognizes that the Partners have a range of organizational budgets (and donor and Foundation funding) and there is no obligation to contribute a specific dollar amount as a financial contribution. In good faith, each Partner will continue to contribute either a financial contribution, an in-kind contribution or a combination of both.
Our Partners are committed to the following guiding principles for the Durham OHT:
- The Durham OHT commits to exemplifying the Patient Declaration of Values for Ontario
- The Durham OHT will provide integrated care through the delivery of safe, quality and connected care to people where and when they need it
- The Durham OHT is aligned to the Institute of Healthcare Improvement (IHI)’s Quadruple Aim of improving population health within the region by partnering to improve health equity and access with a focus on quality, innovation, partnership and patient service, achieving better value by delivering better quality for the same or lower cost, while continuously improving the patient, caregiver and provider experience
- The Durham OHT affirms its desire to work together, in partnership with patients, families and caregivers, to advance integrated care in Durham and to fulfill the Ministry of Health’s expectations for year one through to beyond maturity
- The Durham OHT’s implementation plan is guided first by the needs of the communities and its residents it serves to collectively provide a holistic systems approach to care
- The Durham OHT will continue to build momentum on existing partnerships and projects to realize the full extent of its Vision and beyond
- The Durham OHT will execute co-design principles with patients, families and caregivers in collaboration with service providers
