How Our Program Works
Chronic Obstructive Pulmonary Disease (COPD), which includes chronic bronchitis and emphysema, is a long-term lung disease that makes it hard to breathe. The good news is COPD is often preventable and treatable.

- Individual physical assessment and goal setting
- 12-week group education and exercise session (in person or virtual)
- Personalized home exercise program
- Health and self-management education
- Connecting with community resources
