The Durham Ontario Health Team was 1 of 7 projects approved from the Ministry of Health for the submission of a leading project
- The goal for the Durham OHT Leading Project is for service providers to work together as an integrated team that will enhance care delivery through improving the way team members work and communicate together.

- Inform potential scale and spread of new models of home care, including system processes and supports
- Build OHT and health system capacity for home care planning, delivery, and integration

Eligibility
The Durham Ontario Health Team and Lakeridge Health have partnered with CBI Home Health to deliver customized homecare, dedicated to safely supporting patients and their families at home.
This is a 2-year pilot project and only certain patients may be eligible including:
- Patients living (or receiving homecare) in Oshawa who are over 55 years of age.
Patients who are not eligible, and should be referred to traditional homecare, include:
- Patients awaiting LTC
- Patients who are end-of-life

Requesting Access to General Information
You may request access to general information at Durham OHT under the Freedom of Information and Protection of Privacy Act (FIPPA). FIPPA provides members of the public with a general right of access to information within a health information custodian’s custody or control as of January 1, 2007 (with limited exceptions and exclusions).
Anyone can make a formal request under FIPPA by mailing or delivering a written request to our Privacy Office at:

Request to Access General Records Form
For your request to be considered complete it must meet the following criteria:

Be made in writing and directed to the Privacy Office

Include a $5.00 application fee in the form of a cheque or money order made out to Durham Ontario Health Team

Provide enough detail for staff to locate the information you are requesting with a reasonable search. If we require additional information, you will be contacted
In addition to the $5 application fee, the fees listed below may also apply to your request (as permitted by FIPPA). For each access request received, you will be provided with a fee estimate. If the fees associated with your request are estimated to exceed $100, we will require a 50 per cent deposit prior to proceeding. We will always advise you of any fees up-front, before processing your request.
