Release/Correction of Your Personal Health Information (PHI)

If you would like to request access to your personal health information (health records) or to submit a request for correction to your personal health information, please contact our Privacy Office at (905) 576-8711, ext. 32156, or at homecareprivacy@doht.ca

Requesting Access to General Information

You may request access to general information at Durham OHT under the Freedom of Information and Protection of Privacy Act (FIPPA). FIPPA provides members of the public with a general right of access to information within a health information custodian’s custody or control as of January 1, 2007 (with limited exceptions and exclusions).
Anyone can make a formal request under FIPPA by mailing or delivering a written request to our Privacy Office at:

Request to Access General Records Form

For your request to be considered complete it must meet the following criteria:

• Be made in writing and directed to the Privacy Office 

• Include a $5.00 application fee in the form of a cheque or money order made out to Durham Ontario Health Team 

• Provide enough detail for staff to locate the information you are requesting with a reasonable search. If we require additional information, you will be contacted

In addition to the $5 application fee, the fees listed below may also apply to your request (as permitted by FIPPA). For each access request received, you will be provided with a fee estimate. If the fees associated with your request are estimated to exceed $100, we will require a 50 per cent deposit prior to proceeding. We will always advise you of any fees up-front, before processing your request.
