The core Durham OHT partnership group is comprised of 18 signatory organizations, there is broad support from more than 70 health care partners across Durham Region, representing various organizations in the health, social, education, and private sectors.

Support for Alzheimer’s and dementia includes one-on-one counseling, group supports, caregiver seminars, early-stage education, safety programs (Finding Your Way, Medic Alert) , memory clinics, social recreation, CARERS program, respite services, and adult day programs for individuals and caregivers.

Brock CHC focuses on primary care, family, and community health. Services include treatment for non-life-threatening issues, diabetes education, geriatric assessment, community health promotion, and referrals. Priority populations are youth and older adults, with personal development group support.

Homemaking & Home Support Services, Personal Care Services, Nursing Services, Therapies.

Clarington Family Health Organization (FHO).

Provides therapy rehabilitation services in the community to assist adults and children in maximizing their function abilities within their environments.  The services are delivered by occupational therapists, physiotherapists, social workers and dieticians

Support for Durham Region adults with needs related to aging, physical or mental health and their caregivers helping people remain independent in their own homes for as long as possible.

Health centre focused on family and community health. Services include family medicine, social work and counselling, and a variety of health promotion and community development groups, many focusing on children, youth and women, community-led health teams develop programs to improve the social supports and conditions that affect long-term health.

Grandview offers specialized programs and outpatient support for children and youth with special needs and their families. Services include online parent support, autism tele-practice, tele-therapy, CBT, urgent clinics, virtual assessments, dietician assessments, and medical clinics.

Services include emergency care, surgery, stroke treatment, palliative care, cardiac care, rehab, mental health, women and children’s health, dialysis, GAIN clinic, telemedicine, diagnostics, lab services, and cancer care at R.S. McLaughlin Durham Regional Cancer Centre.

In-home personal support for daily activities, provided by personal care workers, helps individuals with physical disabilities maintain independence. Services are offered at home, workplaces, or adult education centers in the Durham region.

Medical care team focused on family and community health * coordinates patient care in order to assess and treat injuries or illness * team includes family doctors, nurse practitioners, nurses, social workers, chiropodists (foot care specialists), pharmacist and dietitian.

3 Medical Clinics with approximately 145 physicians, including 57 FHO family physicians.

Provides nursing (registered nurse and registered practical nursing) and personal support services to individuals in their homes, schools and throughout the community.

Durham OHT connects local health and social care providers, making services easier to access and improving transitions between providers. The co-designed system aims to fill service gaps, reduce duplication, and enhance the experience for patients, families, and care partners in Durham region.

Saint Elizabeth is a home health care organization.

Co-ordination of all the OHT involvement/engagement of regional programs and services departments. (i.e., Public Health, Social Services, Planning, building and by-law public works and environment.)

Community based home visiting health care and social support services for individuals diagnosed with a life threatening illness and their caregivers * helps people remain independent in their own homes and community for as long as possible * referral to other community services
