Health centre focused on primary care and family and community health. Health professionals assess and treat non-life-threatening injuries or illnesses, diabetes education, geriatric assessment, community development/health/promotion and education, referrals to other local health services and personal development groups. The Brock CHC priority populations are youth and older adults

Clarington Family Health Organization (FHO)
Provides therapy rehabilitation services in the community to assist adults and children in maximizing their function abilities within their environments. The services are delivered by occupational therapists, physiotherapists, social workers and dieticians.
Support for Durham Region adults with needs related to aging, physical or mental health and their caregivers helping people remain independent in their own homes for as long as possible
Health centre focused on family and community health. Services include family medicine, social work and counselling, and a variety of health promotion and community development groups, many focusing on children, youth and women, community-led health teams develop programs to improve the social supports and conditions that affect long-term health
Region providing specialized programs, outpatient clinical treatment and support to children and youth with special needs and their families. * Grandview offers a variety of programs and services including: On-line Parent Support Group, Autism Tele-Practice, Tele-Therapy and Tele-Practice, Cognitive Behavioural Therapy, Telephone Follow-up and Consultation, Urgent Appointments/Clinics, Autism Virtual Assessment, Dietician Assessments, and Medical Clinics
Services include: emergency and critical care, inpatient and outpatient surgery, stroke prevention and treatment, palliative care, inpatient rehabilitation services cardiac care, inpatient and outpatient mental health services, women and children's health, dialysis and kidney care, Geriatric Assessment and Intervention Network (GAIN) clinic, telemedicine clinic, diagnostic imaging, laboratory services, R.S. McLaughlin Durham Regional Cancer Centre
In-home personal supports for the activities of daily living * personal care support worker helps with personal care and assisted living to people with physical disabilities, allowing them to remain independent in their community * services provided in home, at their place of employment or at adult education centers in the Durham region
Walk-in medical clinic with associated family practice * team includes family doctors, nurse practitioners, nurses, social workers, chiropodists (foot care specialists), pharmacist and dietitian
Urgent care walk-in clinic in same facility open seven days a week to provide care
Pharmacy and imaging available during select hours
Medical care team focused on family and community health * coordinates patient care in order to assess and treat injuries or illness * team includes family doctors, nurse practitioners, nurses, social workers, chiropodists (foot care specialists), pharmacist and dietitian
3 Medical Clinics with approximately 145 physicians, including 57 FHO family physicians.
Provides nursing (registered nurse and registered practical nursing) and personal support services to individuals in their homes, schools and throughout the community
Through OHTs, local health care and social care and support providers work as a connected team, making it easier for people to access services and transition between providers, and empowering patients, their families and care partners in their personal health care management. As the Durham OHT, our partner organization, together with patients, family members and care partners, are creating a co-designed system with a vision of becoming one connected system of health and social care and support for residents of Durham region, filling gaps in service, reducing duplication, improving patient, family and care partner experience and demonstrating a successful integrated model of health care provision.
Saint Elizabeth is a home health care organization
Co-ordination of all the OHT involvement/engagement of regional programs and services departments. (i.e., Public Health, Social Services, Planning, building and by-law public works and environment.)
Community based home visiting health care and social support services for individuals diagnosed with a life threatening illness and their caregivers * helps people remain independent in their own homes and community for as long as possible * referral to other community services
