How Our Program Works

Chronic Obstructive Pulmonary Disease (COPD), which includes chronic bronchitis and emphysema, is a long-term lung disease that makes it hard to breathe. The good news is COPD is often preventable and treatable.    

What Our Program Includes:

  • Individual physical assessment and goal setting  

  • 12-week group education and exercise session (in person or virtual)  

  • Personalized home exercise program  

  • Health and self-management education  

  • Connecting with community resources
